Hours80Work ShiftDay: 8 hoursJob DescriptionTo provide quality
pharmaceutical pain management, enterprise wide regulatory
compliance and/or other specialized clinical pharmacy services to
inpatients, outpatients, employees and the general public. To
provide education, information, and advice to physicians, nurses,
and other health care professionals on the use, selection, dosage,
administration of medications, and regulatory compliance . Assists
the Pharmacy Management and Medical Staff to coordinate and perform
TJC required quality assurance activities and Drug Usage
Evaluations. Coordinate and pain management, regulatory compliance
and/or other specialized clinical pharmacy services. Assists with
formulary reviews as requested for P+T committee, assists with the
coordination of staff development activities, evaluates pain
management and/or other specialized clinical pharmacy competencies,
monitors pain management.The clinical pharmacy specialist in pain
management may practice with prescriptive authority for patients
consulted for pain management under the Inpatient Pharmacist Pain
Management Consult Service.Major DutiesPrescribes, monitors and
adjusts medications related to pain management.Obtains pertinent
medical history and obtains accurate pain medication related
history from patient and via chart review.Orders, monitors, and
interprets toxicology screens and other needed labs; implements
follow-up on treatment actions as supported by lab
interpretations.Performs rating scales on pain control, symptoms
and functional status.Provides overdose education and prescribes
naloxone when needed.Provides patient and/or caregiver instructions
and education.Participate as a member of a treatment team in
planning, evaluating, and implementing individualized treatment
programsTreatment authorized under this procedure per the
discretion of the PPMS (Pharmacy Pain Management Specialist) as
outlined in ECH Policy (PolicyStat ID:
7313122)QualificationsDoctorate degree pharmacy (Pharm.D.) from an
ACPE-accredited college of pharmacy.One (1) year accredited PGY1
(Post Graduate Year) pharmacy residency that includes pharmacy
regulatory compliance and PGY2 Pain and Palliative Care Pharmacy
ResidencyOne (1) year experience in compiling medication use
evaluation (MUE) data or equivalent quality assurance (QA) data for
Pharmacy and Therapeutics (P&T) Committee.Minimum of 2 years in
a hospital pain management pharmacist practice or completion of a
PGY2 Pharmacy Residency in Pain
ManagementLicense/Certification/Registration
Requirements/Credentialing and PrivilegingValid California state
pharmacist license with no disciplinary action(s) against the
licensee that prohibits the licensee from practicing pharmacy.Valid
NPI (National Provider Identifier) number issued by the Center for
Medicare and Medicaid Services (CMS)Valid DEA license to have
prescribing privileges registered with the federal Drug Enforcement
Administration (DEA)To meet the increasingly need of delivering
quality and safety care, this position will be credentialed and
privileged upon completion of review on the care providers
credentials and performance and found them satisfactory, authorizes
that individual to perform a specific scope of patient care
services within that organization. Authority is granted based on
establishing that the person has demonstrated competence to provide
these services within the scope of provision of the organization,
and the organization can support their delivery.Ages of Patients
ServedThis position is responsible for the assessment, treatment,
and care of neonate, infant, pediatric, adolescent, adult, and
geriatric patients.Salary Range:$75.09 - $112.64 USD HourlyThe
