Job Description:
Join a flexible Home Care career as a Physical Therapy (PT) Case
Manager, providing skilled home health services in the
community.
Manage your schedule with full-time hours, Monday-Friday, including
an on-call rotation, traveling to clients-------- homes.
Collaborate with clients, families, and healthcare teams to develop
and update personalized care plans, ensuring safety, quality, and
efficiency.
Provide direct care, clinical oversight, education, and coordinate
