difference every day in your work.The Transitional Housing (TH)
Program provides transitional housing to men and women for up to
nine months after completing residential Substance Use Disorder
(SUD) treatment. Clients are supported by Recovery Coaches, SUD
counselors, and behavioral health clinicians, receiving counseling
and education on SUD, life skills, and participating in individual
and group therapy. Our goal is for clients to graduate with skills
and resources for steady income, affordable housing, and
well-being. The Licensed Vocational Nurse (LVN) is a key member of
this interdisciplinary team, supporting client recovery and helping
them navigate the healthcare system independently.Job Summary:The
TH LVN utilizes Nursing Process, Harm Reduction, and Trauma
Responsive Care modalities to advocate for and support clients in
their planning and goals. They focus on helping clients develop
strategies for medication management, including MAT prescriptions
like Buprenorphine, Buprenorphine-Naloxone, and Methadone, through
education within their scope. The LVN collaborates closely with
Addiction Medicine, Medical Directors, and Behavioral Health
Nursing Directors to ensure safe interventions during recurrences
of substance use. They reconnect clients with the MAT team for
routine medication management and perform other duties as
assigned.Responsibilities:
- Function as a nurse within the scope of California LVN
licensure, including credentialing and privileging.
- Collaborate across the HealthRIGHT 360 care continuum.
- Support Medication Assisted Treatment through counseling and
education.
- Monitor medication use and flag high doses or interactions to
prescribing providers.
- Assist in care coordination for clients accessing healthcare
services internally and externally.
- Use the electronic health record for documentation and client
care management.Position Requirements:
- BLS & First Aid Certification, or obtain within 30 days of
hire.
- Ability to work with diverse populations with cultural
humility.
- Prioritize a Harm Reduction approach.
- Interest and/or experience with unhoused populations, SUD and
behavioral health settings, individuals with criminal backgrounds,
or other barriers to self-sufficiency.Individuals with lived
experience and/or fluency in Spanish, Cantonese, & Tagalog are
