Registered Nurse (RN) | Case Manager Location: Santa Rosa, CA
Agency: Compunnel Healthcare Pay: $2,800 to $2,850 per week Shift
Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks
Start Date: ASAP About the Position Note: 2-year acute Hospital
Case Manager experience performing Care Coordination, Discharge
Planning in a very fast paced environment. • Ability to organize
and participate in family meetings. • Participate in the
development of the treatment plan. • Experience with HMOs, IPAs,
etc. • Competent with arranging DME, SNF and ambulance transport.
Must be able to perform this task independently.
