This position is responsible for care coordination and care transitions planning throughout the acute care patient experience. The role involves collaboration with physicians, utilization managers, social workers, and bedside RNs to ensure timely patient progression and transition to appropriate care levels. The Care Management process encompasses effective resource coordination to achieve optimal health, access to care, and appropriate resource utilization. The case manager collaborates with patients, families, interdisciplinary teams, and community resources. This position addresses complex clinical and social situations to prevent unnecessary admissions. Key responsibilities include: Conducting initial and continued assessments of patients. Gathering medical, psychosocial, and financial information to determine readmission risk levels. Developing comprehensive, cost-effective care coordination plans that meet the clinical needs of patients. Identifying quality and risk management concerns for patient safety reporting. Reviewing medical records to ensure patients meet level of care requirements. Collaborating with attending physicians on documentation for level of care support. Monitoring patient length of stay and addressing outliers. Formulating transition plans and collaborating with patients and families on care options. Screening for 30-day readmissions and reviewing previous hospital records. Identifying barriers to care progression and working on resolutions with the multidisciplinary team. Assuring timely transition to lower levels of care. Maintaining communication with financial counselors regarding uninsured and underinsured patients. Developing and maintaining professional relationships with healthcare teams and community agencies. Actively participating in department operations and contributing to the creation of cost-effective practices. Using effective interpersonal and communication skills to promote customer service with diverse populations. Being a positive and engaged member of the team. Requirements Required for Onboarding BLS
