Job Description Physical Therapist (PT) – Home Health /
Outpatient | Day Shift Job Summary:- We are seeking a compassionate
and skilled Physical Therapist (PT) for a Home Health / Outpatient
position based in Hidden Valley Lake (approximately 15 miles from
Adventist Health Clear Lake). This is a great opportunity for a
therapist who enjoys working independently while delivering
high-quality patient care in a home setting. Schedule Monday –
Friday | 8:00 AM – 4:30 PM Optional weekend shifts available based
on need and candidate preference Key Responsibilities Provide
physical therapy evaluations and treatments in a home
health/outpatient setting Develop and implement individualized care
plans to improve patient mobility, strength, and function
Accurately complete and maintain documentation using Homecare
Homebase (HCHB) Perform and document OASIS assessments in
compliance with home health regulations Educate patients and
caregivers on therapeutic exercises, safety, and recovery plans
Required Qualifications Licensed Physical Therapist (PT) in the
state of practice Minimum 2 years of experience (Home Health
preferred) Experience with HCHB (Homecare Homebase) – Required
Experience with OASIS documentation – Required BLS/CPR
